    • - 1. Received from Richard Brazeau DSCP application and check #2531, dated 10/19/2021 for $150.00 for tank replacement at 38 Amherst Road. DSCP#R2021-11-01-A.

    • - 2. Received from Scott Nickerson check #800, dated 10/26/2021 for $300.00 for DSCP Application for 430 Long Plain Rd., owner Scott Nickerson. Assigned DSCP#R2021-10-04B. (Application was approved 10/4/2021 pending fee)

    • - 3. Received from William J. Sieruta, PE a letter dated 10/4/2021 regarding "As Built Inspection for 66 Montague Rd., owner Sandra Susse. The letter stated that the Repair Septic System is in compliance with 310 CMR 15.0. "As Built" plans attached.

    • - 4. Received from William J. Sieruta, PE a letter dated 10/6/2021 regarding "As Built Inspection for 132 Cave Hill Rd., owner Robert Mandell. The letter stated that the Repair Septic System is in compliance with 310 CMR 15.0. "As Built" plans attached.

    • - Citizens Business: Permits and general questions for the board.

    • - 1. Assigned DSCP#C2021-11-01B new construction 3 (Map 3, Parcel 80) Rattlesnake Gutter Road, owner Richard Karsten. awaiting a letter from Health Agent Stephen Ball. Received from Richard Karsten check#5008, dated 10/27/2021 for $300.00 for DSCP Application.

    • - 2. Regarding 83 North Leverett Rd., owner Diane Gorenstein Gina and Health Agent (Steve Ball) witnessed a title 5 inspection at 83 North Leverett Road, Dianne and Tony’s place on Tuesday morning. The septic tank and d-box need to be replaced at a bare minimum. Possibly a low profile system.

      MOTION, moved by John Hillman, seconded by Michael Fair: Motion to allow a low profile septic tank to be installed. .

      Pass: Aye: Michael Fair, Nick Venti, Peter Sylvan, John Hillman. Not Present: Mary Ryan.

    • - 3. Vaccination for 5 to 11 year old's. The school will host a vaccination clinic. The School Committee will be discussing a possible vaccine mandate and would like the boards input. The Leverett Board of Health is not prepared to make a decision regarding mandates for children at this time.

    • - 3. The Committee for the 250 Anniversary asked the Board to advise about the BBQ for the event. This discussion will continue.

    • 7:30: Rich Karsten - DSCP Application review. Map 3 Lot 80 - X Rattlesnake Gutter Rd.
