    • - 1. Received from William J. Siertua, PE an "as built" inspection at 35 Juggler Meadow road, owner John Campbell. The system is in compliance with 310 CMR 15.0 and all Local Board of Health Regulations. As Built plans attached.

    • - 2. Received DSCP Application for 430 Long Plain Rd., owner Scott Nickerson a repair and upgrade (complete system) plans attached. (received $150.00 for Perc. Received a letter from Health Agent, Stephen Ball (see attached) dated 10/4/2021 regarding 430 Long Plain Road. Assigned DSCP#R2021-10-04a

      MOTION, moved by Michael Fair, seconded by Nick Venti: Fair moved to approve DSCP#2021-10-04a on contingent of $300.00 DSCP Fee.

    • - 3. Received COC (Certificate of Compliance) for 259 Long Plain Rd., repair of existing system. COC signed by Designer, Alan Weiss, RS of Cold Spring Environmental and Installer Steve Koniecz of Karl's Site Work, Inc. As Built Plans attached. DSCP# R2021-09-08a

    • - 4. Received from Pamela Veith, check #612, dated 10/4/2021 for $150.00 for Perc Test for Map 1, Parcel 99 Hemenway Rd., (land) owner Gerald Baranoski.

    • - 5. Received from Greg's Waste Water Removal Title5 Inspection Report dated 9/1/2021 for 190 Montague Rd., owner Annette Herda. System passes. Received from Annette Herda check #1263, dated 9/1/2021 for $150.00 for Title 5 Inspection.

    • - 6. Received a letter (see attached) from Health Agent, Stephen Ball regarding 112 Shutesbury Rd., owner Harold Robinson. Revised plan. Variance Hearing scheduled for October 18, 2021 BOH meeting at 7:30 PM. Received from Stoddard Engineering check#675, dated 9/3/2021 for $150.00 for Perc test. (reported received) Received from Stoddard Engineering check #674, dated 9/3/2021 for $300.00 for DSCP Application. Assigned DSCP #R2021-10-04B.

    • - Citizens Business: Permits and general questions for the board.

    • - 1. Update on PFAS testing: PFAS tests have been completed; 4 houses had detectable levels of PFAS compounds. 4 homes from the Teawaddle Hill Road area (where construction of the water pipeline in being installed) had high levels of PFAS compounds. 4 homes around Leverett Pond were samples with no detection of PFAS.

    • - 2. It was reported by Fair that Barbara Bodzin from Life Path that Leverett has received a $10,000 grant for Elder Care.

    • - 3. COVID UPDATE: 1. Using DPH format, less than 5 cases in a 2 week period. 2. Numbers are decreasing steadily.
